Renin-angiotensin-aldosterone system inhibitors – a realm of confusion in COVID-19

چکیده

Currently, there is a persisting dispute regarding the renin-angiotensin-aldosterone-system (RAAS) inhibitors' safety of use in COVID-19 pandemics. On one side, RAAS inhibitors appear to determine an overexpression ACE2, receptor SARS-CoV-2. Therefore, they could increase risk SARS-CoV-2 infection and its degree severity. other discontinuation leads cardiovascular decompensation has been discouraged by major medical societies. Also, large-cohort studies report beneficial or at least neutral effects for patients. Worldwide, millions patients receive treatment hypertension important comorbidities. In this context, knowledge exact effect these medications becomes crucial significance. This paper aims fill gap current presents putative mechanism which inhibitor administration's results can be explained better. beneficial, as counteract excessive detrimental activation classical angiotensin-converting enzyme (ACE) axis, decreasing angiotensin II levels. The blockers (ARBs) levels, while (ACEI) I levels; substrates will compete with ACE2 binding, viral infectivity. addition, following treatment, up-regulated cleave (angiotensin II), particularly 1-7 that possesses vasodilator, protective effects.

Renin - Angiotensin - Aldosterone System

Chronic heart failure (CHF) is associated with endothelial dysfunction. Activation of the renin-angiotensin-aldosterone system (RAAS) is believed to be important in the deterioration of endothelial dysfunction in CHF through stimulation of oxidative stress. Whereas angiotensin-converting enzyme inhibitors (ACE-I) improve endothelial function in CHF, the effects of angiotensin II AT 1-receptor b...

Journal of the Renin-Angiotensin-Aldosterone System (Including other Peptidergic systems) Abstract Since the discovery of a renin-angiotensin system (RAS) in the brain, several studies have linked this central RAS to neurological disorders such as ischaemia, Alzheimer's disease and depression. In the last decade, evidence has accumulated that the central RAS might also play a role in Parkinson'...

Potassium homeostasis and renin-angiotensin-aldosterone system inhibitors.

Inhibition of the renin-angiotensin-aldosterone system (RAAS) is a key strategy in treating hypertension and cardiovascular and renal diseases. However, RAAS inhibitors (angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, aldosterone receptor antagonists, and direct renin inhibitors) increase the risk of hyperkalemia (serum potassium >5.5 mmol/L). This review evaluates the ...
